Combines an app's organisation-level settings with an optional website-level override, backed directly by AppControl and AppSetting persistence rows. Constructed either around a single organisation-level AppControl, or around an organisation-level AppSettings plus a website-level AppControl, in which case website settings take precedence over the organisation defaults.
